Abstract   We herein analysed retrospectively 106 patients with urothelial tumors of the upper urinary tract who were managed at Seoul National University Hospital during a recent 20 year period. The mean age was 58.5 years. There were 86 men and 20 women.The predominant symptom was hematuria(88%). The most common finding on IVP as well as on RGP was fillina defect. Concomittant tumors were found in 25 patients(23.6%), 9 of them had multiple tumors in the upper urinary tract and 18 had bladder tumors, two had both multiple upper tract tumors and bladder tumors. Various surgical procedures were applied in 87 patients and palliative management in 19. Forty five patients were classified in stage pTa or pTl, 13 in pT2, 17 in pT3, and 31 patients had metastatic disease. Sixty eight patients were followed for 6 to 112 months with the mean followup period of 43 months. The 5 year cumulative survival rates of pTa or pTl, pT2, pT3 patients were 96%, 50%, 34% respectively. The mean survival of the Patients with metastatic tumor was 15 months. Subsequent bladder tumors were developed during the followup period in 28%(19/68) of the patients, and tumor recurrence in remnant ureteral stump was 22%(5/23). Prognostic significance of various factors such as age, sex, concomittant bladder tumors, multiple upper tract tumors, stage, grade, previous history of bladder tumor, nonvisualization of the kidney or not on IVP were assessed. Tumor stage, grade and IVP findings had the prognostic significance in univariate analysis(p<0.05), but only tumor stage was significant statistically in multivariate analysis. We concluded that tumor stage was the most important determinant in prognosis.
